What this is, and what it deliberately is not
  • It is a list of questions, not a set of answers. Nothing here tells you what your protein target should be, what your fluid goal should be, or what to do about a dose. Those are the things you are going in to ask.
  • It is not a medical record. Nothing is copied in from the other tools on this site — not your sip schedule, not your pen brand, not your progress log. The sheet contains the boxes you ticked and the words you typed, and that is deliberate: a page that looked like a clinical summary we assembled would be one we are in no position to write.
  • Nothing is stored. It runs in your browser, it is not saved to your device, and closing the tab clears it. Print or download it before you leave the page if you want to keep it.
  • Bringing a written list is a small thing that changes appointments, especially short ones. If someone comes with you, give them the copy — a second person holding the list is how the third question gets asked.
