Stage 3 is the quiet, productive part. The medication is doing its thing on a stable dose, eating has a routine, and the highest-value work shifts to keeping the muscle you have. Two strength sessions a week and protein spread across the day is the whole programme. It is unglamorous and it is the difference between losing weight and ending up in better shape.
Everyone quotes a different protein number, and the most consistent thing in these threads is people telling each other to ask their own clinician for one.

Two is the target. Two mediocre sessions beat one perfect one.

Resistance training is the lever, not cardio
The Nutrients review describes resistance training as central when the goal is supporting strength, physical function and limiting excessive fat-free mass loss, with resistance-based activity performed two to three times per week (Zambrano-Villacres et al., Nutrients, 2026, PMC13258902).
Protein and training work together or not at all
The same review pairs the protein intake range with resistance training rather than presenting either alone. Protein without a stimulus to use it, and training without material to rebuild with, both underdeliver. Pair them.
Fat-free mass loss is a share of total loss, not a fixed amount
Reductions in fat-free mass typically account for about 20% to 40% of total weight lost across pharmacologic weight-loss trials, comparable to other calorie-restricted approaches (Zambrano-Villacres et al., Nutrients, 2026). Faster loss means more of it in absolute terms - another reason the pace is your prescriber's call.
All major muscle groups means all of them
The federal guidance specifies activities involving all major muscle groups - legs, hips, back, abdomen, chest, shoulders and arms (Physical Activity Guidelines for Americans, 2nd edition). A push, a pull, a squat or hinge, and something for the middle covers it in one session.
Protein needs are individual, and kidneys matter
The published range is explicitly adjusted for age, kidney function and other conditions (Zambrano-Villacres et al., 2026). If you have any kidney involvement, the number your clinician gives you overrides every figure on this page and every figure on a supplement tub.

Spread protein across the day
Most people on these medications cannot eat a large protein meal, which makes spreading it the only workable approach. A 2026 Nutrients review puts roughly 25 to 35 g per eating occasion as generally enough to maximise muscle protein synthesis in most adults (Zambrano-Villacres et al., 2026). Three or four smaller occasions, not one large one.
Progress the load, not the duration
Doing the same weight for the same reps forever is maintenance at best. Add a little weight, a rep, or a set when the last set stops being hard. This is the entire principle; everything else is programming detail.
Walk because it is free, not because it burns calories
Walking is the easiest thing to keep doing forever, it is gentle on a body in a calorie deficit, and it does not require deciding anything. Frame it as movement you enjoy, not as a debt you are paying.
Read a body-composition scale as a trend, never a measurement
Consumer scales estimate body composition from electrical impedance, and readings move with hydration, time of day and food. Weigh at the same time under the same conditions and look at the direction over a month. These devices do not measure muscle loss.
Eat enough on lifting days
A day where you train and then eat almost nothing is the worst combination available for keeping muscle. If appetite makes that likely, put a shake or a pouch next to your kit so the decision is already made.

One adjustable pair of dumbbells and a set of bands is a complete home gym for this stage, and it fits under a bed. Buy the increment you will actually progress in - a pair that jumps 10 pounds at a time is useless to someone adding 2. Machines and monthly subscriptions are not the constraint; showing up twice a week is.
